Eligibility:

• Dogs must be 5 months of age or older to attend daycare.
• Dogs older than 6 months must be spayed or neutered.
• Dogs must be friendly with other dogs and people.
• Dogs must be in good health, physically sound, and adequately protected from diseases.
• Dogs must be relatively clean and on a regular flea and tick preventive program.

Fees:

• Single Visit: $26
• 5-visit card: $125
• 10-visit card: $240
• 20-visit card: $440 (best value if your dog comes 2 or more times per week)

Enrollment Procedure:

1. Fill in the Application form below and click Send. This sends essential information about your dog to Dog Gone Happy.
2. Ask your vet to fax to us at (503) 287-5130 your dog's current record of shots. Daycare dogs much be vaccinated against Bordatella (kennel cough).
3. When we receive the Application and the shot records, we will email you to set up a screening appointment.
